Voluntary Short-Term Disability
This coverage provides financial protection in the event you become sick or accidentally injured from a non-work related accident. You can choose a weekly benefit of between $300 and $1,000 (in one-hundred-dollar increments) up to 60% of your weekly salary. The amount of time you must be out of work prior to being eligible is eight consecutive days. Benefits may continue up to 13 weeks. See your plan document for more information.
Voluntary Life & AD&D Insurance
You may elect increments of $10,000 to a maximum of $400,000. Your spouse can elect increments of $5,000 with a minimum of $10,000 to a maximum of $250,000. And your child(ren) can elect increments of $1,000 to a maximum of $10,000. See your plan document for more information.
Voluntary Accident Insurance
This coverage is designed to help supplement your income should you have an accident. Your benefit amount is up to $25,000. Your spouse’s benefit is up to $12,500. And your child(ren)’s benefit is up to $5,000. See your plan document for more information.
